Electronic Record Consent

You consent to receive electronic statements and other records that may be required to be made available for your deposit accounts with Bank of Lake Mills, during the course of your electronic banking relationship with the Bank. This may include, but is not limited to, all legal and regulatory disclosures and communications associated with your account, notices or disclosures about a change in the terms of your account, and privacy policies and notices.

Withdrawal of Electronic Record Consent

You have the right to withdraw this consent by notifying the Bank, in writing, at Bank of Lake Mills, Attn: Deposit Operations, 136 E Madison St, Lake Mills WI 53551, or by sending an email to blm@bankoflakemills.com

Your Right to Obtain Paper Copies

You have the right to obtain paper copies of the statements or other records that were provided to you in electronic form. You may obtain copies by contacting Bank of Lake Mills Deposit Operations at 920.648.8339. We will charge photocopy fees identified in the Bank's current Consumer Deposit Accounts Fee Schedule which may be amended by the Bank from time to time.

Electronic Communication; Current Email Address

Bank of Lake Mills will notify you by email when your electronic statement or other records are available for viewing. The email will include instructions on how to access and view the information. The information may be included in an email or posted on our website at www.bankoflakemills.com.
If your email addresses change, you must notify us by updating your email information on the Bank of Lake Mills Internet Banking web page in order to continue receiving your statement or other records in electronic format. If you fail to notify us of your new email address, we will continue, for a period of time, to send notice that your statement or other records are available to the email address we have on record. You may not receive this notice. The Bank reserves the right to begin sending hard copies of your statement and other records to you by mail.

Computer Hardware and Software Requirements

The hardware and software requirements for access to and retention of the electronic statements and other records were provided to you when you enrolled in Bank of Lake Mills online banking. You will be notified if these requirements change.
